Professional Case Management is seeking a compassionate and skilled Physical Therapy Assistant to join our Healthcare & Medical Services team, providing in-home care for individuals with complex medical needs, including former energy workers and their families. In this role, you will work under the direction and supervision of a licensed Physical Therapist to deliver high-quality, evidence-based therapeutic interventions in clients’ homes. You will implement individualized treatment plans, assist with therapeutic exercises and functional mobility training, and educate clients and their families on home exercise programs, safety, and strategies to maximize independence. The Physical Therapy Assistant will perform detailed observations of clients’ status, document progress and response to treatment, and communicate regularly with the supervising Physical Therapist, nurses, and other members of the interdisciplinary care team. You will help assess home environments for safety risks and recommend modifications or adaptive equipment to support clients’ functional goals. Daily responsibilities may include gait training, transfer training, balance and strength exercises, pain management techniques within scope, and instruction in the use of assistive devices, always following the plan of care and state practice guidelines. This position requires strong clinical skills, clear and compassionate communication, and the ability to work independently in varied home environments while maintaining professionalism and respect for client privacy and dignity. You will collaborate closely with colleagues across the organization, contributing to care planning and case conferences to ensure coordinated, patient-centered services. Accurate and timely documentation in electronic medical records is essential, along with adherence to all regulatory, safety, and infection control standards.
