Professional Case Management is seeking a compassionate and detail-oriented Physical Therapy Assistant to join our Healthcare & Medical Services team, providing in-home care to individuals with complex medical needs. As a Physical Therapy Assistant, you will work under the direction and supervision of a licensed Physical Therapist to deliver personalized therapeutic interventions that help clients improve or maintain mobility, strength, and independence in the comfort of their homes. You will carry out treatment plans, guide clients through therapeutic exercises, assist with functional mobility training, and reinforce patient and family education focused on safety, use of adaptive equipment, and techniques for daily living. In this role, you will visit clients in their homes, assess response to therapy, document progress accurately and promptly in the medical record, and communicate changes in condition or goals to the supervising Physical Therapist and the broader interdisciplinary care team. You will collaborate closely with nurses, case managers, and other healthcare professionals to support comprehensive care plans tailored to former energy workers and their families who may have complex or chronic health conditions. You will be expected to follow all professional, ethical, and regulatory standards, ensuring compliance with state practice guidelines, infection control procedures, and agency policies. Professional Case Management emphasizes collaboration, compassion, and excellence in healthcare. You will be part of a supportive and dynamic work environment that values your clinical judgment, encourages questions, and supports shared problem-solving. New team members receive thorough orientation and ongoing training in home-care best practices, specialized equipment, and condition-specific protocols relevant to our unique client population. You will have opportunities for professional growth through continuing education, skills development, and career advancement within our organization. The ideal candidate is comfortable working independently in the field, managing a schedule of home visits, and adapting care to varied home environments while maintaining a high level of professionalism and respect for client privacy and dignity. Strong communication and documentation skills are essential, as you will regularly update the therapy team, participate in care planning, and educate clients and caregivers on exercise programs and techniques to support long-term outcomes. By joining Professional Case Management as a Physical Therapy Assistant, you will play a key role in helping clients manage complex medical conditions, reduce complications, and enhance quality of life at home. This position offers the opportunity to make a meaningful impact every day by delivering hands-on, evidence-based therapy services in a setting where clients feel most comfortable, supported by an organization committed to quality care, teamwork, and your ongoing development.
