Job Description

Professional Case Management is seeking a compassionate and detail-oriented Physical Therapy Assistant to join our Healthcare & Medical Services team, providing in-home care to individuals with complex medical needs. As a Physical Therapy Assistant, you will work under the direction and supervision of a licensed Physical Therapist to deliver personalized therapeutic interventions that help clients improve or maintain mobility, strength, and independence in the comfort of their homes. You will carry out treatment plans, guide clients through therapeutic exercises, assist with functional mobility training, and reinforce patient and family education focused on safety, use of adaptive equipment, and techniques for daily living. In this role, you will visit clients in their homes, assess response to therapy, document progress accurately and promptly in the medical record, and communicate changes in condition or goals to the supervising Physical Therapist and the broader interdisciplinary care team. You will collaborate closely with nurses, case managers, and other healthcare professionals to support comprehensive care plans tailored to former energy workers and their families who may have complex or chronic health conditions. You will be expected to follow all professional, ethical, and regulatory standards, ensuring compliance with state practice guidelines, infection control procedures, and agency policies.
